Insurance

As a courtesy to you, The REM Center will verify your insurance prior to testing.  This will also include finding out if a pre-authorization is necessary.  At this time the following insurance carriers require a pre-authorization for an IN LAB sleep study:

  • Health Alliance
  • Tricare
  • Aetna - certain plans
  • Humana
  • United Health Care
  • United Healthcare of the River Valley

*This list is subject to change as providers update and change their policies.

We are able to take most commercial insurances; however, there a few we cannot take at this time which include:

  • CoventryIA Health Systems/Trinity Insurances
  • PrincipalIA/IL Medicaid (or subsidiaries)
  • 1st Health Network  Wellmark HMO - Example:  "XQW" prefix gets billed directly to Wellmark
